The 'Ducks in a Row' booklet will allow relatives to find a loved one's personal information and final wishes when they pass.
It has been created by Les Bourgs Hospice, where Liz Stonebridge says the nursing team have first-hand experience of patients and relatives in distress when important documents cannot be found.
Their new booklet, called 'Ducks in a Row', hopes to guide islanders through the process of recording key information, so family can easily access it.
It's a written record if you would like your social media accounts deleted, where are your personal possessions are kept, and what contracts you have signed.
Liz says having the booklet prepared will allow families to focus on what really matters:
"If anyone is receiving end of life care, then that time can be better spent together. Families can try and enjoy those last moments."
The publication launch coincides with Dying Matters Week 2024, which hopes to remove the taboo of death.
"We really want to encourage people to think about death in a different way, and actually be more open to talking about it.
It is something that is going to happen to us all, but as a society we are actually quite nervous."
Copies can be collected, for free, from Les Bourgs charity shop, the Guille Alles Libary and some Doctors' surgeries.
