Dr Mat Dorrian will take over from Dr Peter Rabey as Guernsey's Medical Director.
He will take up the role on 1 December, assuming responsibility for the development and delivery of patient care.
Dr Peter Rabey's retirement was announced earlier this year.
Dr Dorrian is well-known in the island, having been Emergency Department Consultant and a GP at Queen's Road Medical Practice.
He was also a representative of the British Medical Association.
Once agreed by the States Assembly, Dr Dorrian will also become Responsible Officer - having oversight of local clinical governance processes and assessing doctors' fitness to practice.
Dr Dorrian said:
"I will work with colleagues across Health & Social Care, the Medical Specialist Group, primary care and our wider healthcare partners to continue improving services for islanders.
"I’m looking forward to helping shape the future of healthcare in Guernsey for the benefit of all.’
Deputy George Oswald, President of the Committee for Health & Social Care has described Dr Dorrian as 'highly-respected by colleagues':
"He combines clinical expertise with a genuine commitment to collaboration and service improvement, and I am delighted that he has agreed to take on this important leadership role."
His appointment comes at time of intense scrutiny of healthcare in Guernsey, with financial pressures and debate over future-funding, rising demand for services with an ageing population, delays to the redevelopment of the hospital and well-documented concerns about how complaints are handled.
